Description and Services
Description (Service) An in-home respite program designed for individuals with Alzheimer Disease or other related Dementia's as well as physical disabilities. A Coordinator will visit the home and develop a suitable, ongoing program of support, counseling and information/education with the caregiver. Trained Respite Workers are "matched" with clients. Continuity is a priority. Relief time for the caregiver is flexible, based on need, and available 7 days a week.

Eligibility Ages: 18 year(s) and up 
 
Open to caregivers of people 18 years and older who need a respite break from their caregiver role.
Application Referrals may be made by family, agency or physician. Call for an appointment
