Description and Services
Description (Service) Rehabilitation program of Providence Care * System navigation/ service coordination for acquired brain injury services in Southeastern Ontario * a satellite office in Belleville provides rehabilitation support * education *advocacy and consultation for adults who have survived a moderate to severe brain injury. Provides recreation groups for survivors and a support group for family members. System navigation and education provided to those 16+ with a mild brain injury. Community Brain Injury Services provides individualized programs using a participate-to-learn approach. The model rests on roles as goals, learning by experience in real-life contexts and the use of personal and environmental supports to enable participation.

Eligibility Ages: 16 year(s) - 65 year(s) 
Moderate to severe brain injury, medical and neurologically stable. For people with traumatic brain injuries there is no age ceiling.
Application Referrals are accepted from service providers, medical professionals, individuals and family members. Referral form available online:
