Description (Service) Staff provide supports and services for families who have a son or daughter living at home with an intellectual disability and/or diagnosis of Autism * The program offers case management, service coordination,sharing of resources, advocacy support and assistance to apply for and utilize funding including respite * Staff can assist families with long-term planning, making community connections and facilitating inclusion.
Hours Office Mon-Fri 8:30 am-4:30 pm * program hours are flexible

Eligibility Families who have a son or daughter living at home with a diagnosis of an intellectual disability and/or Autism * Birth through to including adults
Application Anyone can make a referral with consent * Families can self refer * Referrals can be made in person, by phone, email or by completing a referral form
