Description (Service) | CV19 Office partially re-opened--call office for update or In-take helpline for referrals * Group activities postponed until further notice Housing supports program designed to help individuals with problematic substance use * helps securing and maintaining safe stable housing * Assist with a rental supplements if needed for variable time periods based on established need * Program consists of a full range of services to support individuals and assists individuals to first stabilize their situation * Later work focuses on harm reduction, developing skills in daily living, developing coping strategies and relapse prevention. Ongoing screening, assessment, monitoring, referral, advocacy, counselling, education and life skills training are provided Program is not an emergency housing program and there is limited availability |

Eligibility | Ages: 16 year(s) and up Eligibility requirements include persons with problematic substance who are involved in or completed an addiction treatment program * have a history of multiple entries in the treatment continuum * Are homeless, at risk of being homeless or inadequately housed * Are unable to obtain or maintain housing without support |
Application | Individuals currently involved with Addictions and Mental Health Services in HPE, KFLA or LLG should discuss their interest in referral to the program with their worker * Referrals from other sources may contact AMHS-HPE to discuss the program and obtain a referral form. |
