Description (Service) Drivers in Ontario convicted of impaired driving or roadside suspension warn range drivers with more than one suspension, must complete this mandatory program * administered by the Centre for Addiction and Mental Health on behalf of the Ministry of Transportation of Ontario and offered by community service providers in a variety of locations across the province

Application Register online * call to have a package mailed or faxed to you, including information on the local service provider * print the registration package from the website * you will need your credit card number and driver's license number * completed registration information may be returned by fax (credit card payment only) or regular mail * register as soon as possible after receipt of suspension notice * no walk-in service
