Description and Services
Description (Service) COVID-19 assessment centre * clinic where people will be assessed by a health-care provider and tested for COVID-19 if required * the test involves taking a sample from the nose with a swab
Hours Mon-Sun 8 am-6 pm
Areas Served Lanark County
Eligibility Individuals with escalating symptoms such as a new or worsening cough and/or fever, and who have either travelled outside of Canada or been in contact with someone who has been diagnosed with COVID-19 in the past 14 days 
Individuals with mild symptoms should self-isolate in their own home until 24 hours have passed without symptoms.  
Individuals with severe symptoms should call 9-1-1 if in distress or go to their local emergency department and follow special signage. Call before arriving if possible.
Application By appointment only * appointments are generation following a referral from Telehealth Ontario, the Leeds, Grenville & Lanark District Health Unit, or a primary care provider (Family Doctor, Nurse Practitioner, or clinic). 
Once given an appointment, patients will drive up to the Main Entrance and remain in their vehicle while an in-vehicle assessment is completed.
