Description and Services
Description (Service) Telephone call to patients who were recently discharged from the hospital for a mental illness * calls will be made within a period of three to seven days following discharge * during the call, the worker and patient will cover areas such as present wellness, medication compliance, safety check and referral follow-up * additional referrals can be provided where necessary * a transfer to the appropriate Mental Health Crisis Team will be made should the patient be found to be in crisis and deteriorating * should the patient be found to be at imminent risk, emergency services will be contacted

Eligibility Ages: 16 year(s) and up 
Patients 16 years and older following discharge from specific hospitals for a mental illness * participating hospitals include the Cornwall Community Hospital, the Hôpital Montfort (Montfort Hospital), the Pembroke Regional Hospital, and the Queensway Carleton Hospital
Application When eligible patients are discharged, they will be offered the option of receiving a Wellness Check call from the Mental Health Crisis Line * written consent from the patient is required before any Wellness Check call can be made
