Description (Service) | Provides a comprehensive multidimensional screening assessment for seniors who are experiencing challenges related to the aging process * aims to: improve the quality of life * promote functional independence and autonomy * prevent or delay hospitalization/institutionalization (unless appropriate) * maintain seniors in their homes within a safe and supportive environment for as long as possible Consider a referral when: recent sudden changes in physical, mental or functional abilities and multiple medications and high risk drug side effects * major changes in support needs, caregiver stress, institutional placement being considered * safety concerns * increased use of health care services * interaction of multiple and complex medical problems |

Application | Family physician, family member, nurse practitioner can refer and a trained Geriatric assessor from the Champlain Local Health Integration Network (LHIN) will visit the client in their home and complete an initial multi-dimensional screening assessment incorporating aspects of physical, functional abilities and environment * collaboration with the senior and family or caregiver helps identify needs and determine appropriate action. An appointment will then be made to one of the clinics to be seen by a geriatrician only when the family physician refers to the Geriatric assessor |
