Description (Service) Single point of access for information and application to developmental services and supports available to adults with developmental disabilities: residential supports * caregiver respite * community participation supports (like recreation, volunteering, employment, or in-home supports) * professional and specialized services * person-directed planning * other provincially funded services and support to help people with developmental disabilities become more involved in their communities

Areas Served Ottawa (City of) ; Prescott and Russell United Counties ; Renfrew County ; Stormont, Dundas and Glengarry United Counties
Eligibility Ages: 16 year(s) and up 
 
Inidividuals 16 years and older with a developmental disability. Visit the website for more information on eligibility criteria.
Application Documentation confirming a diagnosis of developmental disability: psychological assessment, proof of Ontario residency, proof of age is required.
Languages English ; French ; Can provide limited access to translators for languages other than French and English
