Description (Service) COVID-19 (May 27, 2020): Continue to be available to assess patients and address their "essential" needs * may include co-ordination of all health care matters, liaise with hospital and care home staff, translate complex medical information, address complicated issues, direct the effective use of Personal Protective Equipment (PPE), conduct home assessments to decrease hospital visits, and facilitate and/or supervise community-based care

Registered Nurses (RN) with extensive knowledge and experience assist patients and families as their Care Manager for both acute situations and chronic conditions

Holistic approach may include:
* accompaniment to medical appointments
* oversight of treatments and procedures
* communication with physicians and other health care providers
* assistance with access to community resources
* research diagnoses and treatment options
* investigation of alternatives based on patient's needs
