Description (Service) To reach 9-1-1 from VOIP phone

* Call O.P.P. (Ontario Provincial Police) at 1-888-310-1122
* Provide officer with agent information, client information and explain situation

Example:
Hi, my name is (name) and I am calling from 211 Eastern region. We are an information and referral service across Ontario and I have a client on the line from (location/city/town) that needs emergency assistance from 9-1-1. This person is extremely vulnerable, suicidal and can’t hang-up and dial 9-1-1. Can you please send an officer to help this client? Their name is (name), they are located at (address) and their phone number is (number).

Actions:
Giving away possessions * withdrawal/loss of interest in hobbies * abuse of alcohol/drugs * reckless or extreme change in behavior * impulsivity * self-mutilation

Thoughts:
“I won’t be needing these things anymore.”
“I just can’t take it anymore.”
“I wish I were dead.”
“All my problem will end soon.”
"No one can do anything to help me now.”

Feelings:
Desperate * angry * guilty * worthless * lonely * sad * hopeless * helpless

Physical:
Lack of interest in appearance * change/loss in sex drive * disturbed sleep * change/loss in appetite, weight * physical health complaints

Current Factors

1. Are you having thoughts of suicide?

If yes: Safety Plans
* Keep safe
* Safety contact(s)
* Safe/no use of alcohol/drugs
* Link to resources

2. Current Suicide Plan? (How? How prepared? How soon?)
Person is prepared: disable the suicide plan

3. Pain? (Do you have pain that at times feels unbearable?)
Person is desperate: ease the pain

4. Resources? (Do you feel you have few, if any, resources?)
Person is alone: link to resources

Background factors

1. Prior Suicidal Behaviors? (Have you ever attempted suicide before?)

Person is familiar: protect against the danger/support past survival skills

2. Mental Health? (Are you receiving or have you received mental health care?)
Person is vulnerable: link to health worker

When you get a suicide call:
* Ask for name
* Ask for phone numer
* Ask for address
* Once you have confirmed contact info and you have completed your Risk Assessment you will need to explain that you may need to break confidentiality if their life is at risk

If the client is looking for someone to talk to and Assessment Risks are Low (Ideation only):
* No need to break confidentiality; ask for client consent to pass on contact info to agent
* Warm transfer to the Distress Centre/Suicide Hotline/Mental health crisis line
* Introduce client to agent and reason of transfer

If the client is looking for someone to talk to and Assessment Risk is Moderate (Ideation & Plan):
* Explain to client that you will need to break confidentiality and provide agent contact information
* Warm transfer to the Distress Centre/Suicide Hotline/Mental health crisis line
* Introduce client to agent and reason of transfer

If the client is looking for someone to talk to and Assessment Risk is High (Ideation & Plan & Means to Complete Plan):
* Explain to client that you will need to break confidentiality and provide agent contact information
* Warm transfer to 9-1-1 (O.P.P.)
* Introduce client to agent and reason of transfer
