Description (Service) Services and activities include for those affected by Amyotrophic Lateral Sclerosis (ALS): Information and referral * educational program for health professionals * public education * medical equipment loans program * support group meetings for patients and their caregivers * guest speakers provide insight and information on various topics of interest.
Hours Mon-Fri 9 am-5 pm
Areas Served Frontenac County ; Hastings County ; Lanark County ; Leeds and Grenville United Counties ; Lennox and Addington County ; Prince Edward County
Eligibility Individuals diagnosed with any form of ALS
Application Registration with the Society is normally done at the time of diagnosis * Register my emailing the registration form to register@als.com, or fax the form to 416-497-8545 and a Regional Manager will be in touch within two weeks. 
 
To donate to equipment loan program, contact Client Services at 1-800-267-4257, and press 1.
