Description (Service) | COVID-19 Iakhihsohha Home for the elderly and Tsiionkwanonhso: Te are closed to the public. Visit the website for details on all the programs. Client-centered care and services for older adults, and adults with disabilities * help people remain independent in their own homes for as long as possible * culturally appropriate services for First Nations In-home nursing: registered professionals delivering individualized plan of care/treatment In-home personal support: Personal Support Workers (PSW) are trained caregivers who assist individuals with their activities of daily living, including personal care, meal preparation, laundry, and light housekeeping Home adaptation: home maintenance workers assist with minor home adaptations to assist individuals with accessibility and equipment Respite and Palliative: temporary relief for caregivers caring for people with complex medical conditions and end of life care Telephone reassurance: regular phone calls to older adults and people with disabilities who live alone, to check on their health and safety through the Emergency/Medical Alert Units and system Transportation: non-emergency transportation to local medical appointments |
