Description (Service) Intensive mental health case management services, with * limited permanent affordable housing options  
 
* Housing options: Supported/furnished rent geared-to-income apartments for men and women. Individuals in the residential units live independently in the community and receive support and counselling from mental health professionals as needed. Note: * housing is only available to clients currently receiving services  
 
* Mental health family focus case management program offers mental health supports based on the strengths based model, with the individual as primary client * services are extended to encompass the nuclear family where appropriate and approved by the primary client  
 
* The Upstream Model Youth Case Management for students 16-24 yrs  
 
* Psychiatric Assessment Clinic operates 3 days a month
Hours Mon-Fri 8:30 am-4:30 pm

Adults 16-65 years old with a severe and persistent mental illness * Youth Case Management: youth 16-24 years old 
 
Only recipients of Mental Health Case Management Services from a Mental Health Community Support Agencies (MHCSS) partner agency are eligible to access the housing options at this time
Application Community referrals from mental health professionals, doctors, social workers, self or family for mental health case management * individuals should apply through the Mental Health Community Support Services (MHCSS) Central Intake portal housed at the Canadian Mental Health Association - Ottawa Branch at 613-737-7791 
 
* 'The Upstream Model' Youth Case Management : Referral from Algonquin and Carleton University's Health Services
