Description (Service) Access to palliative care expertise  
 
Outpatient service * assists primary care providers in the provision of palliative care to their patients * consultation, coaching, mentoring, and education to primary care providers in the Champlain Region
Hours Mon-Sun 24 hours
Areas Served Champlain
Eligibility Primary care providers, community hospitals with no palliative care services, long-term care homes, retirement homes, nurses providing end-of-life care in the community, and care coordinators in the Champlain region, who need help to support patients with a life limiting disease
