Description and Services
Description (Service) 12-bedroom short stay and emergency respite residence 
A safe and supportive environment where guests can stay for a few hours or 24 hours a day, up to 90 days a year. The service is flexible and can provide emergency or preplanned daytime or overnight accommodations. Unplanned admissions for previous guests are also permitted provided there are rooms available.

Boundaries Champlain Local Health Integration Network territory which includes Renfrew County, the City of Ottawa, Prescott and Russell, Stormont, Dundas and Glengarry, North Grenville and four parts of North Lanark.
Eligibility Applicants must be in the early to mid-stages of dementia, and medically stable. They must not be at risk of harming themselves or others, and not be bedridden or require full assistance with activities of daily living.
Application Contact a Champlain Local Health Integration Network (LHIN) Case Coordinator at 613-745-5525 to ask for the Guest House * If an applicant does not have a LHIN Care Coordinator, ask to speak with the LHIN intake person to arrange for an assessment to determine eligibility 
Once eligibility is determined, a referral is made to the Guest House Program Supervisor. An on-site visit to the Guest House for both the person with dementia and their caregiver will be arranged * The caregiver will assist in the development of an individualized plan of care for the prospective guest * Trial day and overnight stays must be completed before booking additional overnight stays
