Description and Services
Description (Service) Caring and supportive counsel, dementia and caregiver information, education, referrals, and help navigating the health and social care system as well as a social, recreational and caregiver peer support.  
 
Programs and services: 
 
* Dementia Link: telephone-based counselors providing information on dementia, access to dementia education, social and peer support programming as well as local referrals 
 
* Enhanced Dementia Support Program: coaches available to meet in person to support individuals and families as they access the care system and develop strategies for self-management 
 
* Renfrew County Dementia Support Program: support, information, referrals, guidance, education and social programs for caregivers and persons with dementia 
 
* Minds in Action: once-weekly gentle exercise and good fun for dementia partners (caregiver and person with dementia) 
 
* Thanks for the Memories: music therapy offered once a week for eight weeks 
 
* Caregiver Peer Support, Supporting Dementia, and Living with Dementia: various support groups and one-on-one discussions for both caregivers and people with dementia 
 
* Lunch Bunch: social group 
 
* Dementia Basics: public lecture series providing an overview of Alzheimer’s disease and other dementia 
 
* Between Us: education/discussion for young onset dementia patients and their caregiver 
 
* Connections: education/discussion for persons 65 years and older with dementia and their caregiver 
 
* Options for Care: two-part lecture series delivered by the Home and Community Care Local Health Integration Network (LHIN) providing a review of various long-term care options, facilities, eligibility, costs, etc. 
 
* Transition to Long-Term Care: education and discussion reviewing the challenges faced by people with dementia and their caregivers when waiting for placement in long-term care 
 
* Care in the Later Stages: lecture series focused on the condition, care needs, communication strategies and behaviour and pain management for individuals in later stages of dementia
