Description (Service) Rehabilitation and support services to clients affected by a brain injury * includes group activities in Kingston, Belleville, and Brockville * newly referred clients may participate in a group for newcomers to learn about acquired brain injury and identify individual service goals

Adults 18-65 with a confirmed diagnosis of acquired brain injuries.
Application Referrals are accepted from service providers, individuals, and family members * a written Referral Form with medical documentation of an acquired brain injury is required * forms are available from the offices in Kingston, Belleville or Brockville, or can be downloaded on the website * the organization contacts the person within one week of referral

Fees Service - Fees are charged for participants who have access to private rehabilitation funding. ; None - Services are subsidized by the Ministry of Health and Long-Term Care. ; For fee information, call Joni Hartman.
