Description (Service) Serves Lanark Leeds-Grenville Counties: Adult Day Programs (Perth, Carleton Place, Almonte and Smiths Falls, Brockville) * monthly Caregiver Support Groups * individual or family support * Early Stage support * information and referral * educational workshops, seminars and Speakers Bureau * lending library * Finding Your Way * MedicAlert® Safely Home® * Overnight Stay Care program Friday at 4 until Monday at 2:30 (in Smiths Falls) * Minds in Motion: two-hour physical and mental exercise program for the person with dementia and care partners * Brain Wave: education and discussion group for people in the early stages of dementia, First Link Education Series, First Link Care Navigator (help with navigating the system), Minds in Song (choir), Lunch Bunch

Application Apply through the South East Home & Community Care or Champlain Home & Community Care (HCC) for Adult Day Programs * Overnight Stay Care can by self-referral * call to schedule a speaker* Support Groups or personal support call for times or to make an appointment.
