Description (Service) Local chapter of national organization providing the following services: Adult Day Programs (Perth, Carleton Place, Almonte and Smiths Falls) * family and caregiver support groups * individual support * Early Stage support * information and referral * educational workshops and seminars * lending library * Finding Your Way * MedicAlert® Safely Home® * * overnight weekend respite program and Day Programs on Saturday & Sunday (in Smiths Falls) * Minds in Motion: two-hour physical and mental exercise program
Hours Perth Mon-Thu 8:30 am-4 pm or by appointment, Fri 8:30 am-3 pm * Almonte Mon, Tue, Thu 8:30 am-4 pm * respite program Fri 4 pm-Sun 2:30 pm
Meetings Family Support Services for Care Partners of Persons with Dementia - Support groups: 
* Perth: Children With Parents Affected; Alzheimer Office, 115 Christie Lake Rd, (Lanark Lodge), 4th Tuesday of the month 6 pm-7:30 pm; 
* Perth: Spouse Support Group; 115 Christie Lake Rd; 1st Wednesday of the month 1 pm-3 pm, Norm Ferrier Room in Lanark Lodge; 
* Perth: Loss & Dementia Support; For people grieving the loss of someone admitted to LTC or who have passed away, 3rd Thursday of the month 1 pm-3 pm, Norm Ferrier Room, Lanark Lodge; 
* Smiths Falls: Womens' Group; Alzheimer Society site, 2 Gould St. (Elmsley St. entrance), 3rd Monday of the month 1 pm-3 pm; 
* Smiths Falls Caregiver Support Group; Willowdale Retirement Residence 9 Armstrong Dr. 2nd Friday of the month 10 am-12 noon; 
* Almonte: Caregiver Support Group; Alzheimer Society Unit 453 Ottawa St. Unit 4, 3rd Tuesday of the month 10 am-11:30 am; 
* Early Stage Support Group: For individuals with Early Stage Alzheimer's disease or related dementia and their care partners; call central office or toll-free number to confirm dates and times; 
* Individual support: Alzheimer Offices (Perth, Smiths Falls or Almonte): regular office hours or by appointment * evening appointments available 
* Kemptville Caregiver Support Group: office at 215 Saunders St, 4th Thursday of the month 1 pm-2:30 pm

Boundaries Almonte (North) to Gananoque (South) * Kemptville (East) to Westport (West)
Eligibility Ages: 18 year(s) and up 
Individuals affected by Alzheimer's disease or related dementias, their families and the frail elderly
Application Apply through the South East Community Care Access Centre or Champlain Community Care Access Centre (CCAC) * Adult Day Programs, support groups: call the office or attend * education series or workshops: call the offices to let the Education Coordinators know of the attendance * call the office to schedule a speaker * Overnight Weekend Respite is by self-referral with an assesment by the CCAC
