Description (Service) Coordinates mental health services for community treatment order (CTO) application * a CTO is a legal tool that will allow people with severe and persistent mental illness to receive treatment or care and supervision in the community that is less restrictive than being detained in a psychiatric facility

Eligibility Ages: 16 year(s) and up 
Adults 16 years and older with a diagnosis of serious mental disorder; pattern of admission to a psychiatric facility, stabilization, stopping treatment after discharge and readmission 
As detailed in the Mental Health Act * client must have a Community Treatment Order * during the previous three year period, the client must have been a patient in a psychiatric facility on two or more separate occasions or for a cumulative period of 30 days or more * client must have had a previous community treatment order
