Description (Service) On-call assistance to clients living in a home environment
Hours Service Mon-Sun 24 hours
Areas Served Frontenac County ; Kingston ; Lennox and Addington County
Eligibility Ages: 16 year(s) and up 
Individuals 16 years and older who have a physical disability and who:  
* understand the nature of their disability  
* are aware of the type of attendant services required  
* require lifelong assistance  
* are able to direct their own care and can communicate effectively with staff  
* are able to have medical/professional needs addressed by existing community healthcare providers  
* may be left unattended during periods when services are not scheduled  
* have a valid Ontario Health Card
Application Referrals are accepted from the client, hospitals, physicians, social or community health agencies, friends or family
Languages English ; French - by request
Fees Service - Clients are responsible for their own rent and living expenses
