Description (Service) Rehabilitation and support services to clients affected by a brain injury * individualized programs using a participate-to-learn approach * learning by experience in real-life contexts * groups and other activities to learn about acquired brain injury and identify individual service goals * personal and environmental supports to enable participation* outreach, assisted living and psychology services available

Eligibility Ages: 18 year(s) - 64 year(s) 
 
Adults 18-64 years with a diagnosis of moderate to severe brain injury * individuals whose behaviour patterns threaten their ability to remain in the community have priority
Application Referrals are accepted from service providers, individuals and family members. A written referral form with medical documentation of an acquired brain injury is required. Forms are available in the Kingston, Belleville or Brockville offices.

Fees Service - Fees are charged for participants who have access to private rehabilitation funding. ; None - Services are subsidized by the Ministry of Health and Long-Term Care. ; Services provided by Community Brain Injury Services are subsidized by the Ministry of Health and the Ministry of Long-Term Care. Fees are charged for participants who have access to private rehabilitation funding. Clients are responsible for basic living expenses including accommodation, food, and transportation.
