Description (Service) Team that provides assessment, consultation, recommendations, and time-limited treatment * works with the individual, family members, service providers, family physicians and other referral sources to determine a diagnosis and develop individual treatment recommendations to improve the quality of life of a person living with a dual diagnosis * works to maintain individuals in their home and community and to prevent hospitalization

Eligibility Ages: 16 year(s) and up 
 
Individuals who are 16 years and older who have a dual diagnosis: an intellectual disability or autism or pervasive developmental disorder, with a suspected or diagnosed mental illness or behavioural disorder
Application Referrals accepted from all health and social services professionals and family members
Languages English ; French - on request ; Interpretive Services - can be arranged
